Insurance & Payments
Please review the following payment policies for our office.
Insurance
Vitality Chiropractic is not a participating provider with any insurance networks.
Some insurance policies may provide out-of-network coverage, which may allow you to receive reimbursement.
Our office does not bill insurance companies directly. We will gladly provide a super bill for you to submit to your insurance company for any reimbursement available under your policy.
Any reimbursement from your insurance company is paid directly to you. Regardless of insurance reimbursement, the patient remains responsible for payment in full for services rendered.
Payment for Services
Payment is due at the time services are provided.
The patient is responsible for all charges incurred for services provided by this office.
If a patient is referred to another specialist or chooses to discontinue care for any reason other than discharge by the doctor, payment in full is required, regardless of any claims submitted to insurance.
Accepted Payment Methods
Payment is due at the time services are provided.
Insurance Claims / Super Bills
Super bills are available.
Our office does not enter into disputes with insurance companies. If coverage issues arise, patients are responsible for communicating directly with their insurance company, adjuster, or agent.
Any denied or disputed claims will be treated as uncovered services, and the patient remains responsible for payment.
Medicare
In accordance with Medicare law, our office cannot offer special promotions or discounts to patients who receive Medicare benefits.
Collections
Accounts turned over to a collection agency may have a fee of up to 40% added to the balance, as allowed by law.
